PT Claims Data Access Page

  • To access your data, enter your Last Name in the Name Field (it is not case sensitive so you may use all lower case letters), and enter your Tax ID # in the ID Field. Your Tax ID needs to be in the form of  (00-0000000). All dates should be in the format mm/dd/yy, where mm is the month (ex 01), dd is the day (ex 01), yy is the year (ex01). 01/01/01 is January 1, 2001.

-Note: It may take several minutes for the page to load, depending on how much data there is.
